A working, bilingual reporting-flow demonstration created on Future Vanguard's own account for CDC RFQ 75D301-26-Q-00146. It gives evaluators a direct way to inspect the proposed form flow, review experience, accessibility behaviors, and demonstration-only administrative controls.

The site uses synthetic demonstration data and does not submit a report to CDC's production VAERS system. It is a proposal artifact, not an agency deployment or client engagement.

What transfers to contract performance

The evidence demonstrates healthcare-workflow analysis, rapid prototyping, accessible interface implementation, and operating ownership. A contract deployment remains a separate obligation: Government-furnished requirements, privacy and security controls, approvals, and the agency's authorized hosting boundary govern the delivered system.

Future Vanguard's model is a named accountable lead supported by named specialists when the requirement calls for them. Substitutions remain subject to contract requirements and Government approval.
